Illustrative scoring model: each dimension is scored from 1.0 to 5.0 and equally weighted. Green = 4.00–5.00, Amber = 3.00–3.99 and Red = 1.00–2.99. Overall Health Index = arithmetic mean of the eight dimension scores. The overall average must not hide an individual Red condition. Organizations should tailor criteria, thresholds and escalation rules to their governance environment.
